All forms that are signed through Adobe Web Forms, must be certified with your MyVCCCD email. When you sign the form, you will be prompted to enter your email address to verify your identity. Only use your MyVCCCD email. Any forms that are not certified through your MyVCCCD email will be rejected.
Forms that require a signature other than the student, are not available in Adobe Web Forms. These forms must be submitted to your PRIMARY COLLEGE via email.
